How To Submit to the Creativity and Parkinson's Gallery
Please fill out the fields below, attach your work and we will contact you within two weeks about your submission. We currently accept the following file types through the website: JPEG, GIF, MS Word (DOC), PDF. If you have work that involves video or sound, please complete the fields below and email your work to creativity@pdf.org.
A COMPLETED CONSENT FORM MUST BE RETURNED BEFORE AN ARTIST CAN BE DISPLAYED. Download a Consent Form here and return it via mail, fax, or as an email attachment (return information is on the form).
